Home-based Holistic Health Coaching

Brooke will come to your home to evaluate how you live and craft a personalized health plan for you or your loved one. The plan is custom, measurable, achievable, and time-bound.

What the home visit includes

  • Intake and goals check: Brief interview to identify current health concerns, lifestyle goals, dietary preferences, mobility or accessibility needs, medical considerations, and caregiving dynamics.

  • Home environment assessment: Review of kitchen setup, pantry and refrigerator contents, meal preparation routines, dining patterns, food storage and safety, and available appliances. Observation of living spaces for movement patterns, ergonomic or safety issues, and opportunities to support well-being.

  • Daily routine mapping: Walk-through of typical day (sleep, meals, activity, medication or supplement routines, social interactions) to identify realistic points for change.

  • Compassionate listening and culturally informed approach: Respect for traditions, foodways, and household dynamics rooted in the African Diaspora and other cultural backgrounds.

What you’ll receive

  • A written, customized Health Plan delivered after the visit with clear components:

    • Specific goals: Short- and mid-term targets that reflect your priorities.

    • Measurable metrics: Concrete ways to track progress (e.g., servings of vegetables per day, minutes of movement, sleep hours, medication adherence).

    • Achievable steps: Practical, culturally respectful strategies broken into small actions you can start right away.

    • Timed milestones: A schedule with checkpoints (e.g., 2 weeks, 6 weeks, 3 months) so progress is visible and adjustments can be made.

    • Meal-focused guidance: Grocery and pantry recommendations, sample meal patterns and simple recipes inspired by vegan soul food traditions that fit your tastes and abilities.

    • Home modifications and supports: Practical suggestions for kitchen organization, assistive tools, or environment changes to make healthy choices easier.

    • Caregiver notes (if relevant): Roles, task suggestions, and communication tips to support a loved one’s plan.

    • Progress tracking tools: Simple logs or templates to record meals, activity, sleep, symptoms, or vital measures relevant to your goals.

    • Resource list: Recommended community supports, local services, or educational materials.

How it’s measurable and achievable

  • Each goal includes a baseline, a target, and an assessment method (e.g., baseline: 1 cup of cooked greens/day; target: 2 cups/day within 6 weeks; assessment: daily food log).

  • Steps are small, specific, and prioritized so changes are realistic within your daily routines (e.g., swap one processed side for a seasoned vegetable three times a week).

  • Checkpoint schedule allows timely feedback and plan adjustments to keep goals attainable.

Timing and follow-up

  • Standard timeline: Initial in-home evaluation (1–2 hours) followed by delivery of the written Health Plan within 5 business days.

  • Suggested follow-ups: Routine check-ins at 2 weeks and 6 weeks (phone or virtual) and an optional in-home reassessment at 3 months to refine the plan.

  • Flexible pacing: Timelines are tailored to your readiness and health status; Brooke will co-create a schedule that feels doable.

Who benefits most

  • Individuals seeking culturally informed, practical guidance to adopt healthier eating and living habits.

  • Families and caregivers supporting older adults, people with chronic conditions, or others needing a personalized, home-centered plan.

  • Anyone who prefers hands-on, home-based assessment and realistic, measurable goals over one-size-fits-all advice.

Practical details

  • What to have ready: Recent medication list, any medical or dietary restrictions, and a quick tour of kitchen and living spaces.

  • Confidentiality: Assessments and plans are handled with respect for your privacy and household dynamics.

To schedule an in-home evaluation or ask about accommodations, contact Brooke with your preferred days and a brief description of household needs.

⭐️⭐️⭐️⭐️⭐️ Five-Star Review
Brooke Brimm, a Minister of Mind, Body & Spirit and Holistic food healer, came to my home health agency and provided exceptional care to one of my very kind clients—who is truly like an adopted family member to us.

She was patient, loving, and deeply caring—physically, mentally, and emotionally. Brooke treated him as if he were her own family, with respect, compassion, and genuine dedication. She didn’t just offer support; she provided clear, thoughtful steps to improve his overall well-being and helped enhance the direction of my business at the same time.

Sometimes all you need is a roadmap and she left us with exactly that. She created a clear paper trail, actionable guidance, and a renewed sense of hope. She met my client exactly where he was and helped redirect his health, mindset, energy, and outlook on life.

If you are someone who needs to start over, reset, reprogram your body and mind, improve your health, or even refocus your business and family life, Brooke Brimm has the tools, knowledge, and heart to help guide you forward.
